Overview
This template is designed to help people managers conduct comprehensive annual reviews. It includes sections for setting goals, tracking monthly and quarterly progress, and providing final ratings.
Sections Explained
Employee Information:
– Fill in the employee’s name, position, department, and review period at the top of the sheet.
Annual Goals and Objectives:
– List up to five goals for the employee.
– For each goal, specify the objective, target date, status, and measurement criteria.
Measurements:
– Define Key Performance Indicators (KPIs) for each goal.
– Track the results of these KPIs throughout the year.
Monthly Reviews:
– For each month, document the employee’s achievements, challenges, feedback, and score.
– Use the “Score” column to rate the employee’s performance for that month.
Quarterly Reviews:
– Summarize the monthly scores for each quarter.
– Document overall quarterly achievements, challenges, and feedback.
– Calculate the quarterly score based on the monthly scores.
Final Yearly Review:
– Aggregate the quarterly scores to determine the final yearly score.
– Use the rating criteria to assign a final rating:
– Exceeds Expectations: 90-100
– Meets Expectations: 70-89
– Needs Improvement: Below 70
– Provide comments to justify the rating.
Employee Comments:
– Allow the employee to provide their feedback on the review process and outcomes.
Signatures:
– Include signatures from the employee, reviewer, and 1-on-1 reviewer.
– Add date fields next to each signature for record-keeping.
Using the Template
- Setting Goals:
– At the beginning of the review period, set clear and measurable goals for the employee.
– Ensure each goal has a specific objective, target date, and measurement criteria.
- Monthly Tracking:
– At the end of each month, review the employee’s performance.
– Document achievements, challenges, and feedback.
– Assign a score based on the employee’s performance.
- Quarterly Reviews:
– At the end of each quarter, summarize the monthly reviews.
– Calculate the quarterly score and document overall achievements and challenges.
- Final Yearly Review:
– At the end of the review period, aggregate the quarterly scores to determine the final yearly score.

- Employee Feedback:
